I need SMART GOALS for my DNP PROJECT which is on Advanced Directives and Living Wills in a Senior Center. My plans are to identify these patients on admission and retrospectively going through their charts for the ones I have to do retrospectively . I know many admission assessments have , do you have an advanced directives ? as a question to the patient. My goal is to identify these patients and educate them and their families on getting information on Advanced Directives and the ultimate goal is they eventually decide to have an advanced directive or living will written up. I also want to offer this to Spanish Speaking patients. My ultimate goal is to make sure patients are educated in getting informed that their medical wishes can be left in writing for their loved ones to follow if they were to fall into a coma and not be able to make these decisions for themselves.
This has to be written in a table (like one you can make in Word and list the 5 SMART Objectives on the left column that I propose for my project and the second column the outcomes.
I am planning on starting to gather my information September 2023 and by the end of December 2023 get some information on the data I have collected. I will have to get the employees from the facility involved , perhaps the director of nursing , the charge nurse, patient family. There may be a questionnaire as a tool to use that is yet to be created.
